and I mean really seriously screwed up. What do you recommend?

I don't know if this would classify as really messed up, but I can only tell from my experience.

'Brain' by Robin Cook, a medical thriller, is one messed-up book I have read. The plot is good, but the author really messed up the structure of the story and its narration. You wouldn't believe this, but the author uses the first name and the surname of the character interchangeably at places, confusing the author. Like, at one page it's like "Martin did this" and at some other it's "Philips did that", Martin and Philips being the one and the same person, Martin Philips, the protagonist. Hardly the mistake you would expect from a bestselling author.

As I Lay Dying, by William Faulkner: family drives mother's rotting corpse by horse and wagon 100+ miles to lay her to rest. Each chapter, the narrator is a different member of the family -- each with a varying grasp on reality.

Read "A Feast of Snakes" by Harry Crews. Really messed up, but excellent.
