Order of Succession
According to the Presidential Succession Act of 1947, if the President of the United States is incapacitated, dies, resigns, is for any reason unable to hold his office, or is removed from office (impeached and convicted), people in the following offices, in this order, will assume the office of the President, provided they are qualified as stated by the Constitution to assume the office of the President, which means they have to be must be at least 35 years old, must be a natural-born U.S. citizen, and have lived in the U.S. for at least 14 years.

Secretary of State
Secretary of the Treasury
Secretary of Defense
Attorney General
Secretary of the Interior
Secretary of Agriculture

Secretary of Energy
Secretary of Education

The 25th Amendment of the U.S. Constitution, passed in 1967, provides for procedures to fill vacancies in the Vice Presidency; further clarifies presidential succession rules.

The question you are asking is not chain of command - but the order of succession. These are 2 different things.
The first link I gave you is on chain of command, which is a is a top-down summary of hierarchy as used to give and distribute orders in the military branches of the US. (Pecking order, if you will.)
The second one is order of succession - which is basically who becomes the commander in chief should all those above somehow be incapacitated.

the new VP as appointed by the old VP now Pres, and I'm guessing, approved by the congress
only if the VP died before a new VP was appointed would the Speaker become Pres
