I do to others as I would like to be done by them... so my flawed being has to give many, many chances. A true second chance includes forgiving and returning the count to zero!
Some times as a flaw, my ego has denied a loved one a chance or has kept a little resentment within me giving a false second chance without forgiveness... in the end that has been terrible for all.

Not really. I know maybe I should but people have done some really bad things to me in my life (Ive made my mistakes too) but some of the things people have done to me are so bad that its not worth giving them a second chance. It’s like a waste of time. I will forgive people but letting them take up time in my life again is asking too much :) lol At the end of the day, its just me anyway so why let people keep coming in my life and continue on to make the same mistakes that hurt me?

Usually a second and sometimes even a third chance, but if they insist on being total unrepentant repeat jerks, I move on and rarely look back.

Yes, I'd give as many chances as the person needs. That is
as long as they have not harmed a child or one of my children.
Children are sacred and, my children will never be less sacred as far as I'm concerned.

Yes, usually, 2nd and 3rd chances, and I end up being hurt as usual.

There is only a few things right off the top of my head that will get someone booted,,cheat or lie(relationship)steal or cause harm to me or loved one(anyone else) all but cheating is circumstantial--

if money or anything other things on earth can buy a chance. once chance slip by who knows when one will get such a chance ever again, right?
