If you are sure that it is not a fan, back up all data. Then buy a new disk and obtain disk cloning software to copy the old disk before it fails completely.
I used Paragon Partition Manager 2005 to clone a WinXP disk that was making grinding noises. This version appears to be free to download via:

It may be getting ready for it's last spin.
What you can do, buy another Hard Drive and set it as the Master Drive and set the old one to Slave. Install the XP to the new drive and use the old as a backup/storage drive. The old drive may last for years like this, but, do not put very important stuff on it. I would probably use it for Diagnostics stuff that can be Downloaded again should it bite the dust.

This could also be a fan going bad. Make sure you clean all the dust out of your computer, if you have not lately. This is good to do in general, a cool computer is a happy computer.
This is a suggestion. If you know that the sound is coming from the hard drive, then its time to backup your needed files. It may be going bad.

m having the same prob for some time now,the sounds cumin out of the hdd is bad news.....the only thing to do is to get it checked by a company tech,if its in the warranty period ,otherwise bad luck.....n the sounds occur only ven any prog is accessing the hdd....so jus check yr task manager(press ctrl+alt+del) to bring it up,n end any unwanted processes....
