Is it your stomach or your abdomen that hurts?
If it's your abdomen, it could be that you are contracting those muscle, causing them to hurt. If you don't think that's the problem, then you could have something wrong with your appendix or a hernia. If it persists, I would call your doctor.

The answers would be different based on masturbation or sexual activity, but a generalised non-medical diagnosis would be strained/sore muscles

I bet you have a hernia. If its a small one, it will show on a ultrasound.
