Who are the four heads of house and which house do they belong to? No guessing and please reference the chapter and book that gives the info.

Professor McGonagall is head of Gryffindor, which is mentioned throughout the entire series. Snape is head of Slytherin, which again, mentioned throughout the whole series. However, Slughorn becomes a stand-in Head of house of Slytherin at the end of book six (I don't remember the title, but it's while the heads are discussing the fate of Hogwarts) Sprout in the head of Hufflepuff. I can't remember what chapter its in, but I know she's mentioned as head in the 4th book, because it was mentioned that she was Cedric's head of house. Which leaves Flitwick for Ravenclaw. Never specifically mentioned, but in the same chapter Slughorn represents Slytherin (book 6 discussing the fate of Hogwarts) there are 5 teachers in the room: McGonagall, Slughorn, Sprout, Flitwick, Hagrid. We know Hagrid isn't a head, and we know what the other 3 are heads of, so that leaves Ravenclaw for Flitwick.

Off the top of my head, Professor McGonnigal is head of Gryffindor, Snape is head of Slytherin, & I believe Professor Sprout is head of Hufflepuff. I can't think of the Ravenclaw head right now. I'd imagine this info is all in the Sorcerer's Stone.
