it is total bases divided by At Bats, number of hits has nothing to do with it, someone could have a double and an out and be 1.000 SLG or someone else could have 2 singles and still have a 1.000 SLG
A walk does not count as an At Bat, but it does count as a plate appearance, this one of the main differences between on base percentage (times on base safely/plate appearances) and batting average (hits/at bats)
sac fly, and hit by pitch are both not counted as AB

Correct. When a player is walked, hit by a pitch or hits a sacrifice, those do not count as an at bat, and therefore would not be counted in slugging percentage calculation at all. If a player goes up to bat 4 times, and is walked once, and gets a hit once, he would be 1 for 3. Bases/official at bats.

So if you went 3 for 3 with a single, double and triple that is 6 total bases so it would (6 / 3 = 2.00 Slugging %)

Yes, it's total bases per official atbats, meaning walks don't count in your Slugging, nor do reaches on errors, hit by pitches, sac flies, etc. One injury plagued year in softball, I had nothing but singles so my batting average equaled my slugging percentage, as I got 1 base per X number of at bats. If one were to hit nothing but home runs, his slugging percentage would be four times his batting average.

Its pretty simple... you know that batting average is the number of hits divided by the at bats. Slugging percentage is the total number of bases attained (i.e one for single, two for double, etc) divided by the at bats. A perfect slugging percentage would be 4.0 (all home runs every time!)
