Throw harder and practice an hour or two a day. Sounds like a lot, but that's the only way to achieve near-perfection. You'll notice that after probably just one short practice "session," you are pitching faster than you were before. However, you also need to move your body faster in order for the pitch to move at a quicker rate. Now that you have your aim down, it's just a matter of tying loose ends together to perfect your pitch like you want to.
Hope this helps, and good luck! ;-)

One way a pitcher I know, who throws very hard, works on throwing harder is to go to a gym and kneel by one of the pads along the baseline of the basketball court and throw the ball as hard as she can into it. I sit next to her, giving her balls, so it's "rapid-fire" throwing. This is good because you not only work your pitching arm muscles but you build up endurance, as well.

What I would do is in addition to practice, practice, and practice, also work on wrist flicks. Much of your speed comes from flicking your wrist, so try wrist flicks from different spots, such as 2 feet away from a target, or the full 40 ft. I promise your speed will increase!

to throw harder you have to make your arm and leg go faster, so really try to explode off the rubber. drive with your leg, don't push of the leg that stays on the mound but drive with the other. make your arm go as fast as it posibly can.
just throw hard.
