Update: http://history.nasa.gov/SP-350/ch-13-1.html has former Astronaut and NASA executive James Lovell asserting that there were no suicide pills on space missions. Lovell is best known as one of the astronauts on the infamous Apollo 13 mission.

Nasa came up with a better idea than suicide pills . The Apollo missions all had a tv set on board tuned to Canadian television . Each astronaut also had access to a razor blade .
If the astronauts found themselves in a hopeless situation halfway between the earth and the moon , they`d simply turn on the tv set to one of CBC`s channels . After 30 seconds of CBC , they`d want to reach for that razor blade to end the torment .

No. I've met Apollo 12 astronaut Alan Bean and he said that NASA made it very clear they would have to die trying to fix the problem, they would not have an alternative.
In the Apollo missions it was also unlikely they would have been able to talk to their families as that would be admitting defeat.

No. Though some pilots (like CIA pilots, etc.) are said to carry cyanide capsules for that purpose, the astronauts do not.
