where cole/belthazor dies for good? as in never shows up again in the series.

centennial charmed. it was the show's 100th episode and cole's 117th birthday. he became an avatar so that he could turn back time, right after prue died and before paige showed up. he did this, believeing that if it wasn't for paige, he and phoebe would have ended up together. however, paige sneezed and orbed as he was turning back time. when she orbed back, it was in a different reality wherein p3 was closed, piper and leo are divorced, phoebe and cole are married but they're both having affairs, and with that reality's paige dead. she convinced her sisters that she is also a charmed one, and just in time, she managed to convince piper and phoebe. cole, not having the powers he had when he became an avatar, was vanquished.
but, julian mcmahon (cole) guest starred in the 150th episode, the seven year witch. piper was hanging between life and dead, and she was stuck with cole.

After realizing that he will never get Phoebe back in this reality and getting tired of her sisters' constant attempts at vanquishing him, Cole agrees to become an avatar. The first thing he does with his new powers is throw himself into an alternate reality where he is once again Belthazor and Phoebe and Piper never met Paige, meaning the Power of Three was never reconstituted. Paige herself is thrown into the opposite reality also when a sneeze makes her orb during Cole's spell. While Cole attempts to live his life happily with Phoebe as his bride, Paige tries to convince her sisters that she is their long lost half sisters and get them to help her vanquish Cole once and for all. The only problem is no one believes her claims!

Centennial Charmed - this is where the Avatar incarnation slips into an alternate universe where he and Phoebe are still together but Page has a cold so she sneezes and is alive when in the time line she is supossed to be dead. He dies on his 117th birthday.
