Is it possible the first surgery was done wrong?

It probably wasn't done wrong, it may have been the wrong treatment. The numbness and pain from carpal tunnel comes from the median nerve. The medial nerve originates from several nerve branches in the neck. When you pinch the nerve you seldom get the sypmtoms right at the point of compression, they usually happen distally or away from the sight. I recommend that you get a chiropractic evaluation. The doctor can pinpoint exactly where the problem really is and treat it conservatively and maybe avoid another surgery.

Yes-quite possible the transverse carpal ligament was not fully cut. This happens if the person doing the surgery is not familiar with the anatomy of the hand. I would ask another doctor to operate on me.

You are doing something repetitively. Is it in only one hand? Are you on the computer a lot. If so, get a thumb activated track ball mouse. They take some time to get used to but relieve the wrist from right-left mouse duty.

If the first surgery helped then it was done right. Don't repeat the actions that cause it.

no / carpel tunnel /does come back after surgery /sometimes / it is caused by work / that is repetitious / if need be / get a job that /is not that / good luck
