Most international teams will have at least 4 lineout jumpers - two locks (second row), the #8, and the blindside flanker. In some cases the openside flanker will also act as a 5th jumper when required. These are generally the tallest men on the field.
The hooker doesn't have to call the throw, as has been suggested in an earlier answer. In fact, the hooker doesn't even have to throw the ball in. Back in the old days, it was commonplace for the winger to throw into the lineout.
A team normally designates a "pack leader" who will make the lineout calls........

A jumper in the lineout basically can be anyone of the forwards, (unless you wanna put a back in it which is also fine). it is not an official name but basically is just the guy who will be lifted to catch the ball in a lineout in a designated play called by the hooker (wearing 2)
usually "jumpers" are the locks wearing 4 and 5 but in recent years "loosies" namely forwards wearing 6, 7 and 8 have been used as "jumpers" as well, cos they are getting taller over the years.

Usually the 2nd and 4th men in the lineout are the 'jumpers' and they are usually the locks who are usually the tallest players. However sometimes they throw the ball to the back of the lineout to the #6 or #7 man in the lineout.

Hmmm, well I can tell you that I play wing [lots of scrum half too though, but they lift me when I am winger] and on our team, what happens most often is that they choose the closest back, usually the inside center, outside center or wing.
I guess what you could draw from that is, I don't think there is a name for that position.
The player that throws the ball in is the hooker though ;)
OH! and another thing with regards to plays during line-outs, sometimes we will tell the hooker to throw it directly to the prop instead of the jumper or even the halfback ;)

It started out with locks jumping in the line-out; nowadays anyone in the lineout (except the props) is nominated as the jumper. Props would be supporting the jumper.

The jumper in the line-out hasn't an actual position name. But he will be one of the forwards.It's usually one of the flankers as they are the tall ones.

there is no official name, they are usually refered to as the jumper.
It is usually 1 or 2 people and they are usually the tallest forwards, but to keep the team guessing they will switch them round.
They will even sometimes throw the ball right over the line out and the half back will catch it.

there are calls for the players in the line outs so one of the players can jump in the front ,middle or the back
