For some outstanding martial arts/swordplay action, check out Samurai Champloo. It's a short series, but a lot of fun, and great action. What's more fun that watching two skilled fighters take on a restaurant full of assasins with no weapons beyond a pair of chopsticks and a bowl? :)
For great creative action/fighting, I have to say Full Metal Alchemist (it is also my favorite anime, overall) The mixture of hand fighting, weapon fighting, and alchemical fighting is both exciting and visually stunning to watch. Great stuff.
I also must mention Avatar: The Last Airbender. I don't think its actually considered anime, and its target audience is pretty young, but I am 30 and I love it. The different fighting disciplines (air, earth, fire, water) are each modeled after 4 different martial arts, and the combination of actual martial arts moves, with the different elemental powers makes this show a 'must watch' (and its got great humor and story lines as well...)

You want action? Yu Yu Hakusho. That show revolves around violence. >D Hehehe.
As far as action goes, Inuyasha has decent fight scenes... but as for the actual storyline... meh. It's a bit lacking, in my opinion, but hey that's just mine. ^_^'' (sorry all you Inuyasha lovers out there XO lots of love)
Uhm... Black Cat is just good... I don't know about the anime... but the manga had some action in it and the artwork is lovely.

Right now im watching Basilisk - has a lot of action fighting scenes. Hellsing was pretty good, Trinity blood, oh and im watching Blood+ this is a good one also. Full Metal Alchemist has alot of action and fighting.

watch One Piece, even though its over 300 freakin' episodes long, the action scenes are crazy insane.
First of the north star, even though the show is old and the story isn't all that great. The action is what drew its appeal

If you want HORROR/GORE/ACTION/NUDITY(upper body) all in one:

It'll keep you on the edge of your seat! Could be the scary anime you're looking for!

>>>ELFEN LIED-14 eps- A declonius is dangerous, two horned mutant with telekinetic powers...too bad one escaped from the lab. Two cousins on the beach find a naked girl, take her home, and name her "Nyu". Turns out that Nyu is actually Lucy, the creature from the lab. Better watch out, cause Nyu has amnesia, but you don't want to be around when she turns back to Lucy!!
Love this one . . . "nyyuuu!"
If you want ACTION/COMEDY all in one:
>>>FULL METAL ALCHEMIST- 52 eps + movie- Two brothers Ed and Al on a quest for a stone that will repair the body of Al, botched by an attempt to bring their mother back to life.
Loved it-the characters really grow on ya!

Well if you like a lot of gore + action + cool graphics and story check out claymore. It's an awesome anime with alot of action.
Oh and bleach is pretty cool too. It's both humorous and actiony. You can see both at veoh.com. Youtube's deleting anime.

Fullmetal Alchemist is good. Mabie D N Angel.
As for scary, perhaps Iggie Pop Phantom. Elfen Lied freaks me out, but you should only watch that if you're 18 or so. (Rated M)

I have to agree, avatar is a pretty good show even though it's not anime. naruto's good, but not alot of action. the new season is pretty much all about talking =) the anime that pretty much tops it all in my "favs list" would probably be samurai champloo. awesome anime. samurai deeper kyo is pretty cool too
