The emergency brake is located in the rear rotors. You have to pull the rotor off to get to the e-brake pads. They may just be worn down and won't hold.

You will have to first inspect the brake cables to see if they are working or broken. If the cables look okay, you will have to dis-assemble the rear disc brakes. You will have to remove both rear calipers and then pull the rotors off. Inside the rotors is a emergency brake drum. The emer. brake is a set of brake shoes. Inspect and repair as necessary now. The rattling might be associated with the rear brakes, but I kind of doubt it as you say it's from the middle of the truck. To me it sounds as if your transfer case needs servicing as the fluid inside here was changed to a different type and one of the things that changed was it needs to be periodically flushed out as it tends to cause a weird rattle under conditions that you have described. Good luck.

Well it depends what is wrong with the brake. It could be a problem with the cable or cables going to the rear wheels or it could be that you just need the e-brake lining kit replaced.
