You only pay somebody for a domain name if you want a domain name. There's millions of people at home connected to the internet. Do they pay for a domain name?
A server is just a computer "serving" something like a web server, DNS server, FTP server, etc.. Any computer can do this, although "servers" tend to be robust and specifically built.
The point is, to be on the internet, your server just needs an internet connection (pay your ISP, or with a real server, one of the Tier 1 or 2 providers), and you get an IP address.
However, if you want a name, like example.com, you need to pay an accredited registrar to use that name.
Does your server require a domain name? Ask yourself, can people who visit your server make do with an IP? For example, gaming servers frequently *don't* have a domain. Yup, just IPs. Many people run an FTP server. Or SSH. Or whatever.
But with public HTTP servers, for webpages, you don't want the public to be typing in IP numbers. That's why a domainname is common.

You do have to register your domain name once a year. Usually that is done for you by your host as part of their service. But you can do it yourself. Registering the host name is what allows your IP address to be used as a domain on the Internet. The fee (last time I checked a couple years ago) was like $35 a year.

Yes you have to buy your domain name from yahoo or godaddy or whatever domain and point it to your server by adding it on dns in your server.
moreover in the domain control panel you should put your IP in Name Servers.

It depends on wether or not you want to put the server on the internet or not. There are plenty of sites out there that will register your domain name for free, but if you want visibility for you site you will want to register with a good company.

Yes.
Someone has to take responsibility for providing the routing from the internet to your server and they'll expect some money for it.
