3 years is entirely do-able for an undergraduate degree. Most colleges on the credit hour system require 120-130 credits to gradate. You would take 15 credits per semester (that's 5 classes) and 12 credits per summer semesters (4 classes). So 15 credits * 6 reg semesters + 12 credits * 3 summers = 126 credits.
It's not that crazily hard, as 15 credits per term is not even considered overload. Summers are tough , but short, so it's not too bad.
One limitation is your college and major program. You need to make sure it's large enough such that pretty much all classes are offered year round. Small schools sometimes only offer a course once per year. That course could be a prereq to something else and that'll totally kill any chance of early graduation so be careful.

Absolutely. It's possible to do it without them, but much easier if you come in with a semester full of credits. I finished up a semester early, and a close friend graduated a full year early with the help of a large number of AP exams. Some schools give you credit for 3+, some for 4+, and some only for 5's.

what took me 2 1/2 years of graduate school for a masters
another guy from my high school did it in 1 1/2 years
I took full loads of 12-14 hrs - he consumed 18-21
and did marvelously
you have got to be able to live in the library - and forget a social life
it can be done

Yes - one way of doing it is taking courses in the summer. One of my former students when to college and was able to finish his four year degree in three years for example.
