The fourth dimension is usually time, however in theoretical physics there can be a 4th spatial dimension. The only problem being that we couldn't percieve the 4th spatial dimension. It would be like a person who lives on a sheet of paper in 2 dimensions trying to perceive someone or something that exists in 3 spatial dimensions. Their brains can't percieve it since their whole realm of reality is strictly 2 dimensional. For more info on 4 spatial dimensions check out Hyperspace by Michio Kaku, he gives some excellent illustrations and examples of multiple spatial dimensions.
As to telekinesis, that is a phonemonon usually found in people who have some kind of background in the occult. In that light the fourth dimension takes on a whole different meaning, but that's a different subject altogether.

Time can be considered a fourth dimension but not a physical dimension like the three we know. As far as telekinesis, don't bet your house.

I have heard of machines in the machine shop that can cut in 4 dimensions, and even some that cut in 5 dimensions. I have never seen one myself but would love to have the chance to use one someday.
Maybe its not the samething your talking about but still, its pretty cool.

Einstein believed that it was time.
Spatially speaking, (on par with the first three; height width and length) other's say that it is orthogonal (perpendicular) vectors attached to an object.
