He wore #12 against Orlando in 1996. Coincidentally, the reason he wore it was that his #23 was stolen before the game. I saw that game on WGN too.

In addition to 23 and 45, Jordan wore #12 one night against the Orlando Magic after his jersey was lost or stolen.
When Michael Jordan retired from Basketball, NBA retired his #23 Jersey. When he decided to come back in 1995 he wore #45 (as his jersey was retired). After one year, while playing against, Magic Johnson, Johnson made the remark that Jordan didn't look like himself with #45. So he once again donned his #23 jersey even though he was fined. He only wore the #45 for one year (1995 season).
He wore Number 45 when playing for the Chicago White Sox also
