Jones Town. David Koresh and the Branch Dividians. Russian and China under communism.
Both Jones Town and the Branch Dividians failed because both leaders were demogogues. Jones told his people the world was going to end and they were going to suffer persecution and he should be the one they should follow. David Koresh said he was Jesus Christ.
Both communist countries failed because the pepple in power get the priviliges. The common people have to barter and trade since money is useless and common goods are hard to come by. Blackmail is rampamt.
The advent of a little capitalism is allowed to thwart any open need for the common people to want truel liberty and freedom.

I think that the Soviet Union would be the biggest example of that. Marxism was supposed to be a utopia.

Works of fiction that discuss this are Farenheit 451 and 1984, although I know this is not the answer you are looking for.
