A few things immediately come to mind. Adam and Eve eating that apple. Emperor Nero burning his own city. Winston Churchill's World War I invasion of Turkey at Gallipoli. Hitler and Mussolini declaring war on the United States 4 days after Pearl Harbor. How about the release of thalidomide in the 1950s? Arguably also.. finding OJ Simpson not guilty and releasing Paris Hilton from jail early.

A really good question and I have thought about it, but I guess we all our own worst decisions, not earth changing, but lifetime changing.

Here's a few I can think of.
Hitler violating his non-aggression pact with Russia, before finishing off Britain, thereby forcing Germany to fight on two fronts.
Hitler not leaning from Napoleon's mistake and trying to beat the Russian winter.
George Armstrong Custer drastically underestimating Lakota and Cheyenne warriors at the Battle of the Little Bighorn.
Titanic reducing the number of lifeboats on board to limit clutter on the top deck.
No one acting on U.S. intelligence agencies information prior to 911 that could have stopped the attack. Similarly, ignoring warnings about Japanese threats prior to Pearl Harbor.
Bishop Diego de Landa burning many thousands of Mayan codices.

I think one of - there have been many - the worst decisions was when the English sold Joan of Arc back to the French who would later execute her by burning her.

Neville Chamberlain slapping the hands of Hitler as Hitler gobbled up Europe. England and France could have stopped the Reich by having gotten involved earlier than they did.

Depends on your point of view. If the Japanese had continued to attack the U.S. fleet after Pearl Harbor, or had gone ahead to attack the mainland, they probably would have destroyed us and won WWII. However, they drew back, and went home. That translates into a bad decision for Japan..

Slavery. importing (by force) black natives from their natural home to a land they did not know anything about. Still worse, depriving them of their freedom and use them as machines. This is a worse thing that ever happened to humanity.
