I give them a shock. My reply to that question is "I feel terrible" they then stop in their tracks, stare at me and then ask. "Oh that's too bad and what is the problem? I then reply and explain to them, If I said everything is good, I would not catch your attention. Most people agree and say that with an automatic response like that their attention would not even raise an eyebrow. But with a response like this one does catch ones attention.

If its in business its an automatic fine, or great. If its family or my best friend I tend to be more honest because I know with them I won't be misunderstood.

If it's a casual acquaintance I say would say fine thank you. If it's someone close I would tell them if I am fine or not doing so well.

Because the question itself is simply rhetorical and really doesn't require anything more than a simple acknowledgment...
I just say, "I'm fine, how are you?".
