You can't actually record four outs. You could say you give a team 4 outs by having an error. Also you could have a dropped third strike and have the runner reach first. Because of this you could have 4(or more) strikeouts in an inning.

It's not really possible, but it can appear that there is a fourth out made. Typically this requires an appeal play -- runner left the bag too early, perhaps -- after the third out is recorded, in order to nullify a run scored. If the appeal is successful, the "fourth out" is upheld, the run annulled, and the previous third out reverts to a man left on base.
This happened with Mike Pagliarulo (his run counted, as the defensive team did NOT appeal) about 20 years ago, I described it in an answer a few weeks back, but the search engine isn't cooperating.
The fourth strikeout, when a dropped or missed strike three allows the batter to reach base, is not the same thing. The pitcher still gets credited his strikeout but the putout (which is credited to the catcher) is not made. Different scenario.

You don't.
It's possible for a pitcher to get 4 strikeouts in an inning, but not 4 outs. Once the third out is recorded, the inning is over.

Every inning has a total of 6 outs....three for each team.
Therefore it is possible to get 4 outs and have the game called because of Rain. Ta-Dah...4 outs in an inning!

You can't officially record four outs. A pitcher can have four (or more) strikeouts if he gets a strikeout for each out and one (or more) of the third strikes are dropped by the catcher and the runner gets to first safely. I have also seen a double-play turned with two outs but, again, they only officially record the first out.

An infielder catches a pop-up while standing on the base when the runner didn't tag up with 2 outs. Thus the batter is out on the pop and the runner is out at the base at the same time.

3rd strike becomes a pass ball and the runner reaches on an error. It is a stike out but not an out. Then the pitcher can record 3 outs and have 4 outs.

Get two doubles 1 makes it two outs and then make another so it would be four outs but it is only recorded as three outs and then you can get a triple or a single.
