The FJH Classic Scale Book is really good. I've used it for ages! It has all major and minor scales in contrary and parallel motion, triads and inversions both blocked and broken, arpeggios, and cadence chords. Plus there are instructional pages and practice guides

It's been a long time, but I believe the book was called something like "Czernsy: Scales and Velocity Studies". From what I can remember, it was very good for scale studies, fingerings, and speed increasement.
