He was killed...in fact, the ending was genius if you've paid attention to the show or are just a fan of well developed well thought out plots that all tie together and have the memory of a champ to remember it all. The ending was simple: He got killed! Let me tell yall why and explain in detail...
There was 3 people in the room total who had a reason to kill Tony: The two black guys...they were paid before to kill Tony but he was only shot in the ear, this was in one of the earlier seasons. Also in the earlier seasons...the trucker who was sitting at the bar stool, who the camera kept focusing in on, is Nikki Leotardo, Phil Leotardos nephew, he was in one of the early season episodes where Phil and Tony have a sit down....
Here's where the genius comes in...
When Tony's walking in the diner,you see the camera focus on him, then it switches to his perspective, and you see him looking at the booth he's gonna sit at... then the camera switches back to Tony's face, then it once again switches to his perspective, and it shows him looking at the door and looking at the people come in..... Everytime the door opens the Chimes sound....... Carmela walks in, Chimes, AJ walks in Chimes, all this when Meadow is parallel parking, still trying to get inside the restaurant.... at this point the camera switches back to the trucker who goes into the bathroom...
Then it goes to a scene where Meadow finally parks and starts running in the diner... the door's about to open, Tony looks up....and...
No Chimes - - - -
No Music - - - -
Everything just goes black - - - - -
In one of the early episodes of the Sopranos, Tony's talking with Bobby about what it must feel like to die; Bobby says "at the end, you probably dont hear anything, everything just goes black"
Part of that was revisited in the second to last episode during the last seconds of it, when Tony's about to go to sleep and he flashes back to the memory of him and Bobby on the boat... "You probably dont hear anything everything just goes black."
So in the end, the Journey song was playing, the Chimes on the door sounded but when Meadow came in, the guy in the trucker hat came out and killed Tony... It's the reason you don't hear, or see sh*t when he died.... it was from his perspective.... where everything went black, then the credits rolled.

The end was saying that even though the big threat had been neutralized (nice touch with the car rolling over Phil's head, too) that he will always be the head of the mob family, and that while life does continue to go on for him and his family, that he's never going to be safe. He's always going to have to watch for hitmen, or the law, or someone trying to get him, to one up the top guy. Notice how they focused on every guy in there, that Tony had to keep an eye on?

It was a cold slap in the face. Everything was being set up to look like Tony Soprano was going to get gunned down in his onion rings. The guy peering from the bar gets up to go to the mens room to get the gun from behind the toilet; the two other guys walk in the front door right at that moment, Tony looks up, and the screen goes black.
It's like being right at the point of orgasm and getting a horrible cramp in your leg.

Everything was left open. They made it ambiguous on purpose. Maybe Tony got whacked, maybe he got pinched, maybe the two black guys and the one white guy going to the bathroom were just random people. You don't know. Looking back, I actually think it was the perfect way to end the show. Nothing was resolved. Either you can draw your own conclusions as to how it ended or they can spin it into a continuation of the series or maybe even a movie.

It means it leaves the producers/creators an opportunity to cash in later on a blockbuster movie or HBO movie. $$ in their pockets. Does James Bond ever die? Neither does Tony Soprano.

at the end of the previous episode, tony and bobby were talking about what it's like to die. "you probably don't even feel it."
at the end, that guy keeps staring at tony. he gets up and goes to the bathroom so he can approach tony from behind. meadow taking so long to park means she's not in the seat next to him, so tony's perfectly exposed. meadow walking in is the last thing he heard, and then darkness.
david chase said he's always known how the show is going to end. the only thing that makes sense is that tony's dead, and he never saw it coming.

The ending was not what I thought it would be. In my opinion, they wanted everyone to use their imagination as to what the ending would be. In my opinion, I think the man who left the bar stool & went to the bathroom, would come out of the bathroom & shoot the Tony, his wife, & kids., & that's why the screen went black.

It means David Chase got tired, You get old and get sick, end up in a state hospital. It means life goes on. It means that
there is going to be a next season. who knows what it means besides the people who wrote it.

They want to make money later, and keep the possibilty open for another season later or a movie. It's all about money. Books, interveiws ect..
