You should've talked to the manager (without being rude) about what was going on. There's a difference between being busy (and having a lot of customers) and being busy (and having a few customers, but not a lot of staff available). Either way, "we're busy" shouldn't be an excuse for the shoddy service. The wait to be seated doesn't bother me as much as the long wait for food. 15 minutes to get an order taken can't be quite long, but that really depends on how long you were looking at the menu also. Plus, 20 minutes to get food isn't too bad for a busy day or night, but still, the server should've at least let you know they were backed up in the kitchen and that your food was on it's way. Now, by getting the manager to get your food, that should've alerted them to "oh! My staff isn't doing what they're supposed to be doing!", but since it obviously didn't, here's what you should do:
I have found that for most chains, it's easy to email the corporate office. If they have a website, go there, and find the "contact us" section. Be specific as to the date and time of your visit, and everything you listed here. Tell them in your message exactly what happened, and even mention the manager didn't seem to care that you weren't happy.
At the least, the chain should send you an apology letter, but you might get lucky and get a gift card to come back for a free meal. We went to a movie theatre in our area a while back that serves full meals. We waiting 30 minutes before our order was taken (and another 15 until we got drinks). We got our pizza about 15 minutes into the movie, blazing hot, but they didn't bring us plates (it was on a serving platter) and only one square coctail napkin between two people. Talk about being pissed! We even asked for plates, were told that they don't bring plates out for the pizza (but if we wanted them, they would bring them for us). We asked for the plates, and more napkins, and never saw either the rest of the movie, even asking several times. I ended up emailing the corporate office and recieved a letter with 2 free movies tickets, and a ticket for a free order of popcorn. Needless to say though, we haven't been back, and that was a year ago.

I would call the restaurant and request to talk to the owner or corporatte office. If that is not possible, I would write a letter to the manager or owner, nicely explain your experience. I would add that "word of mouth" is the best advertising and that when people have a good experience, statistically they tell 4 people, if it is bad, they tell 20. If that information is relayed in an appropriate manner, it usually gets their attention.

If it is a chain restaurant, contact their home office and let them know about your experience. If it is locally owned, the manager you spoke to might not be the general manager. I would ask to speak to that person. Hopefully, you will get some resolution from either of those options. If not, definitely don't go there and warn others you know about your experience.

Go to the website for the company you want to complain on. Usually they will ask for your phone number and address and call you later. They usually send out coupons and whatnot for a bad experience. Sorta to compensate for it. I would email the company personally. I have done that a few times.

If the manager won't help, go up the food chain. Regional VP, owner, CEO and complain. You should at the very least be "comped" on the food.

I've never had a decent response from a chain store yet and boy, have I made some complaints!

If it is a chain get the number of the location (should be on your recipt) and e-mail or call the corprate office. Don't hold back

I think you can put reports like this on a site called "angie's list". that is used for reporting service issues with businesses, both good and bad, but you do have to name names.
