Eye's sticking out is a symptom of one of several diseases and is commonly called pop eye. If both eyes are sticking out it's most likely the very first sign of dropsy. This can also be due to a liver infection (both the dropsy and the pop eye).

To treat this condition will require antibiotics. The best available over the counter treatment for this is Furanace, Furan-2 or similar compounds containing Nifurpirinol. Another effective treatment is nitrofurazone which is found in Furazone among others.

If you have access to a vet that can and will write prescriptions for you, Cipro is a very effective treatment as well.

The other possible cause is the beginning stages of Lateral line erosion or Hole in the head disease. This responds well to the the first 2 treatments as well and it's quite reasonable to assume it would respond well to Cipro as well, but I have seen no confirming tests on that.

There is a totally undocumented "disease" to be found on one website called saucer eye. This is supposedly a disease only found in Flowerhorns that causes pop eye and is treated wth nothing more than a change in diet. Again there has been no documentation of this supposed disease. Please consider ignoring this as a real and possible diagnosis and treat with effective medications before the real cause becomes too difficult to treat.

The condition your fish has is pop-eye, or more accurately known as exophthalmia. He will lose his eyes if not treated.

The condition is usually from environmental factors, like undesireable water conditions. It might be from too small a tank (a FH should have a 50 gallon or more) or a poorly maintained or filtered tank, or an overfed tank.

It is a bacterial disease and should be treated with a good ani-bacterial med right away. Remember that this medication will kill the good bacteria too. If you can, preserve your filter media in a container of water with ammonia added to it daily. Make sure all the conditions are right for the fish so it doesn't happen again.

I've dealt with pop eye in Flower horns and Oscars several times in the past.

I've always used Maracyn-two by Mardel Labs or Mela-fix, with several times of water changes.
