what is the fix for this and roughly how much does it cost?

Are you sure the floor has risen? In most cases, concrete floors slabs will sink, not rise. A proper garage floor pour, will include some good sized 'tubes' of concrete to be poured under the floor, to below frost line, for added support of the floor, with a vehicle.

Sinking floors and foundations can be fixed. Look for a company that does "mud jacking" in your area. They can cut into the concrete, install some special equipment, and then pump a concrete slurry under the existing concrete, and bring it back up to level. This is usually cheaper (and easier) than trying to bust up a whole floor to replace it with new, hehehe.

This is a very hard question to answer without some specific information. The best bet is to contact a contrete contractor and ask for an estimate. Most of the reliable ones will do an estimate for free. Be prepared to pay in the neighborhood of $15 per square foot to have the old concrete removed and a new floor poured.
