A pedicure. I'm not kidding. Get a gift certificate for a pedicure (might as well throw in a manicure as well) at the nicest salon in your area. Get a business card or brochure with the hours, etc. Make it expensive enough (perhaps a package deal and that might even include a facial which would mean a nice shave too...) that he won't toss it aside.
A lot of guys have never had a pedicure but it is a real treat. Next time he needs to clip his nails, he will let them do it for him and he can spend an hour or two being pampered.
Why did I think of this? Because a friend who is a senior citizen was clipping his nails and did a bad job of it - and had to have surgery on his toe! It's a chore. Why not make it a treat?

Is he diabetic? If not - CHOCOLATE! :)

A lot of times guys just don't treat themselves to those little extras that women often receive like flowers and chocolates. If you have the money, order him some really nice chocolate and have it delivered. There is a great list of chocolate distributors at http://www.chocolate-candy-mall.com/chocolate-candy-sellers.html

If you are a bit strapped for cash, make up your own homemade chocolate candy and present it in a nice looking box or tin or even on some unique platter. He may enjoy the homemade even better knowing that you took the time. There is a variety of simple but really good recipes at http://www.chocolate-candy-mall.com/homemade-chocolate-candy-recipes.html

As mentioned before, be sure to let him know how much you love him.

offer your services. help do those big jobs that he knows have to be done but is dreading doing. senior dad's really don't want all the goofy gifts that they can't really use. my dad would much rather i helped him out in the garage and treat him to lunch than buy him something he has no use for.

Make him a photo album with all his kids any grand kids in it.You could even tie in a theme with the album, such as fishing or golfing if that's what he likes.I had a friend of mine who made her dad a DVD photo album complete with his favorite songs and pictures. He really liked it allot.I'm not sure where she got it made, but you can probally find a place to do it in the phone book or on-line.Good Luck.

A heart-felt letter from you, with a photo. The one thing men want more than anything else, and this is all men, spouses and brothers and sons included, is to be appreciated. If you can send him a letter, with a photo of you and of what you hold dear, and explain to him how much you appreciate him, then that's the single most valuable thing you could ever do for him. He'll love you for it, and he will cherish this gift more than all others.
