I have read some other possibilities that he wakes up in a dream..maybe from still in a coma of being shot or that fake character he played is actually him. He was just dreaming the whole mob boss thing
If I were a betting man, Phil gets taken out early so they concentrate on the rest of the show. Paulie will get some kind of promotion...Tony will step down in some way. I don't know if it is Witness Protection or something like that. I think the final scene will be with Tony and his family enjoying some kind of positive moment with Phil being the only one taken out!

I think Paulie is going to kill Tony. He is very disgruntled and I think he was the one to warn Phil and family to get out of town when the cousins from Italy shot the wrong guy the "goomah" sp??. If you watch the episode a second time, you will catch what I mean.
That is what I think.

Tony & Family in a small house in Arazona under witness protection. His family (the real one) was threatened & he had to turn to save them.
