From a travel agent whose traveled extensively throughout Europe here are my suggestions:
If you're planning to travel around Europe, plan out where you are going, keep it reasonable for the amount of days you'll be in Europe, then purchase a Eurailpass. A real bargain, but you have to purchase it in the US - not available in Europe.
Most European cities have incredible public transportation - if you think gas in expensive in the US, multiply it by 4 and you have a good idea of what the Europeans are paying. Even if you have a car you only drive it when you absolutely have to. Bus, streetcars, and subways are very safe, easy to use and inexpensive. If you plan on doing a lot of moving around in a city, ask about a city pass (less than purchasing individual tickets) that will allow you unlimited travel for the days you'll be there.
All cities have some kind of tours, so if you want to get to know a city, ask about a motorcoach tour at your hotel.
You can also book private tours, with a car and driver, that will show you the sites and be at your disposal. This is the best way to maximize your time and see a lot of a city if you are not spending a lot of time there. However, this will be the priciest option.
If you've never been to Europe before, you may just want to consider an escorted tour with Tauck, Trafalgar Tours, or Brendan Tours. This way you can see a lot and not have to worry about anything.
Whatever you decide, the best thing you can do for yourself is to use a travel agent to set all of this up for you. Don't try to do it yourself. You're spending a lot of your hard earned money and want to make sure you see and do the things you're interested in - so let the agent set all that up for you.

Depends where you are, most major cities have great public transportation. Whether subways, trams or buses. I don't get lost often, however when I do it's always one of the better parts of the trip. When you are lost you often see things you wouldn't other wise see. It's great to get lost, I wouldn't worry about that.
You travel between cities usually by train as Europe has a great train system. You can always rent a car, that works very well too. I have driven around large parts of Europe and it's easy to get where you want too go even not knowing the local language.
Don't worry you will have a great time.

Trains will get you just about everywhere you need to be. Check out a Eurail pass if youre going to be travelling quite a bit and long distances. Theres also discount airlines such as ryanair.com and easyjet.com. Within just about every major city there is efficient public transportation. Whether it be trams, buses, or a subway system, you will be able to get around fairly easily. At the major trainstations, there should be an information desk with attendents that can speak english to help you get started and give you timetables etc. Its also helps to do a bit of research online before you go so you are somewhat familiar with how to get around in each city.

Europe has an exellent train and coach system which is relativly good value for money , although some coaches i have been on have been a little dodgy lacking decent air condition ect
Im british and dont speak any foreign languages exept for french and have always got to where i needed to go relatively easily.
Most people are friendly and speak a little english and there are plenty of tourist info centres where you can go if you get really stuck.
Just enjoy the experience , sometimes getting a little lost is all part of the fun of travelling.
