I think you need to go to an agency to get an agent, and then once you have the agent, you become part of the agency.

You need a separate agent for acting and modeling.
Being in an agency or having an agent is the same thing. There are independent agents who work alone or have their own agencies and there are bigger agencies who have a number of agents working under their umbrella. Everyone has their own opinion as to which is better, a small boutique agency or a larger one.
A smaller one may give you more personal attention (but maybe not, it depends on the agent) and some say you can get lost in the shuffle at a big agency, however some projects are cast fully in house at big agencies. If a writer's script is optioned and his agent is at CAA, they may also get their director and entire lead cast from CAA. Personally I think that it's the agent, not the agency. Meet with them and between the ones that think you have potential and want to sign you... decide who you have the best rapport with. Go for a trial period of 3 months or so and see where you are at the end of that. If you're both happy, sign for a year!
This is more for acting. For modeling, that is not my area. I only know of larger agencies but I do know that Ford, Elite, etc. have information right on their site about how to become a model with them. Follow their advice to the letter and keep trying! They have open calls often so if something doesn't go right one week, you can try again the next.

The Talent Agents work at The Talent Agency, they are employees of the Agency. If you want to model, contact the modeling agencies in your city.
When you get a talent agent, he or she will work at the Talent Agency.
A Talent Agency is where Talent agents work, their office is located in an office building.
If, you have an appoint with a Talent Agent, you go to their office building where they work.
