8 X 10 and your resume is either printed on the back or stapled back to back with the photo (cut to match the size) so they can just flip your pic over and see all your info. Any copy shop can crop your resume for you, in fact they can do all the cutting & stapling! Just be very specific on what you want.

8x10, black & white and steer away from wearing a turtleneck.
Some also use a collage of photos (dramatic, comic, classical, casual) but still in the confines of an 8x10.
Having your resume printed on the back is a great and handy idea. Always bring more than copy to an audition.

8x10, in portrait orientation. It has to go into the same file as your resume, so it needs to be similarly sized. At one time there was a fashion of actually printing the resume on the back of the headshot, but I don't think anybody's doing that any more.

We hardly look at them but we usually look at age range, height, weight. The thing is we can see your photo too, so if you run out of room you don't NEED to have that info on there.
