Another option is if you have a copy (or access to a copy) of Microsoft's Developer's Studio.
I haven't tried it, but I believe that GIMP can do it (and also a decent amount of what most people use Photoshop for). All for Free.

Experiment. Download a free trial of Adobe CS (http://www.adobe.com/products/photoshop/index.html) or Corel's Paint Shop Pro (http://www.corel.com/servlet/Satellite?pagename=CorelCom/ProcessLayout&lc=en&ppg=CorelCorp/Trials/Login&pid=1156169694605&cid=1156169693940). Those are the two programs I use the most, though there are more. There are dozens of icon tutorials, especially on Livejournal itself too if you browse (or 'explore') for 'icon tutorials'. Best of all, have fun and try new things. There is an entire world to icon making, including textures, brushes, backgrounds, images, etc, all of which can be found if you search.

Download an IconMaker program.
They're easy to find, but hard to find a permanent free one.
Or you can use something like Paint and just export it as an .ico
