Ronin is right, but also you keep your agent on the lookout for those projects that you are most interested in. If you don't have one, then you have to look for those projects. They don't come up often, but when they do they need smoeone who has really studied their craft and knows some good tricks! You can't be reckless either, you must know all aspects of safety because they will likely be counting on you for expertise especially in lower budget projects. Just be friendly, open, and know your stuff!

Keep checking sites such as actorsaccess.com and backstage.com if you don't have an agent or even if you do and you weren't sent out for a certain project you can call and ask to be sent on it or you agent may tell you why he/she didn't send you on that particular film.

Both. Mostly hard work. The guys who work with Jackie Chan for instance work with him all the time to be stuntmen. You need to be very good at what you do.
