In my experience, the best thing we did with our blended family was to take things slow and let things fall into place naturally. Never, ever force the kids to hang out together or to love each other like family. Counseling was also good for us too. After a few years when things were better, I adopted my wife's kids (through rapidadoption.com) and things were solid from there on out. I am not saying step parent adoption is best for every family, but it worked for us too after a good few years of getting used to each other.

Show patience, do not rush or force relationships to form, but let them form on their own. Also, family counseling can work too.
