No - it is blended with past and present tense, here is the correction into past tense:

There was an old woman quietly praying at the corner of the church. She wore a black veil on her head. The old lady who was holding the rosary and praying at the altar gave him a very blank stare.

not consistent at all... you want it present or past tense?

present tense:
There IS an old woman praying at the corner of the church. She wears a black veil on her head, and prayS quietly.
The old lady IS holding the rosary and IS praying at the altar. SHE STARES AT him blankLY.

past tense:
There was an old woman WHO prayED at the corner of the church. She WORE a black veil on her head, and she prayED quietly.
The old lady HELD the rosary, prayED at the altar, AND STARED AT him blankLY.

how about that?

The second sentence should read:

She was wearing a black veil on her head and was praying quietly.

It's grammar friendly, but not consistent in your tenses. You start with the past tense: was, but then switch to present tense in the second sentence: wears, and present progressive: is praying. In the third sentence, you go back to past: was, gave.

nope, you fluctuate in your tense usage, when writing essays or something, keep them in present tense:

try this: An old woman prays near the corner of a church, she WEARS a black veil and holds a rosary in clasped hands as she prays quietly...

as far as the "he" is concerned we don't know who "he" is you need to specify who "he" is, we don't know who you are referring to.

KEEP IT IN PRESENT TENSE OTHERWISE YOU WILL HAVE A STRING OF PASSIVE SENTENCES=BORING!!!!!

and its TENSE, not TENSES
only use tenses if you aren't grouping it with something else , like uh GRAMMAR AND TENSE HELP, well your SUBJ-VRB agreement must be aligned. help is your verb and GRAMMAR and TENSE is your subject.

THERE are 3 basic VERB TENSES, well you couldn't say there are 3 basic verb tense, unless you say verb tense forms, either way there is plurality.

Grammatically it's fine as long as you have Noun phrase and a verb phrase you are fine, having a subject and predicate basically.

Like, "Ice Melts" subject- Ice Pred- Melts
