I use a program by HobbyWare called Pattern Maker. You may import a graphic into the program and it converts it to cross stitches. Scan your picture first and save it as a jpg file.

You will want to do a lot of cleanup as you most likely will want to "merge" close colors and get rid of "strays".

Go to http://crochetdesigns.com/software/. The program will more than pay for itself when you start creating cross stitch and filet crochet (and tunisian crochet) designs. I've used it to make name lace, portraits, and personalized afghans. You can import any jpeg file and it will convert it to a grid form. It doesn't give the specific name of the colors, but you could print it and take it to your local craft store and compare the colors in the graph to the colors of the thread or yarn you are using.

Another idea that I heard was to print your picture out on graph paper. That would give you an idea of what you're working with I think.
