That's a swim bladder infection. It will be hard to treat at this point but if you want to try I would suggest Furanace or Maracyn TC. Just treat as directed on the package.

Constipated.Swimbladder treatment needed.80% of the time
they die in 6 days.Stop feeding it.

It is slowly dying. Something is wrong in your system if this fish dies. Too much bacteria, etc. Find out to prevent it in the future.
