That's a swim bladder infection. It will be hard to treat at this point but if you want to try I would suggest Furanace or Maracyn TC. Just treat as directed on the package.

It is slowly dying. Something is wrong in your system if this fish dies. Too much bacteria, etc. Find out to prevent it in the future.
