i just took compounding part 3 and was wondering what is considered a fatal error in the exam. is using a wrong bag concidered a fatal error if it does not effect the stability of the drug? and making a capsules that are over wight more then 10% how many points will be deducted for that? please help me figure out if i am gonna be a NY pharmacists or Connecticut pharmacisit?

There are various things that can be considered a fatal area like not checking if the person is on another medication beside the currently prescribed medication that could lead to harming or killing the patient.

A fatal error is something which could lead to harming a person. Over/underweight capsules, wrong label, wrong directions, wrong amount of drug etc are considered fatal.
I'm from IL, but we did discuss major errors in compounding.
