For any document to be legally binding, it must be written by a lawyer. You can try reproducing a disclaimer that is used by an organization that does the sort of thing you want.

Disclaim what? They have not to die when on tour or if they do they will be left at the roadside....or they fall at their own risk....what age limit are you thinking of? Only the under 60's need apply...
Are you expecting someone to sign this disclaimer, or just have it on the ticket....
Never heard of such a thing...nor would I tour with someone who advertised such a disclaimer...and I'm not elderly.
