I always leave 10% unless I really have run them around ragged with special requests or if they were just so exceptional. In those cases I go a tad more.

According to the website I've listed in sources, between 5-10%. Of course, that is completely up to you how much you tip. I've worked in both buffet restaurants and in cook to order restaurants, and I can tell you, 75% of people don't tip enough. Not that I'm biased or anything lol..

I tip at buffets but only if the service is good. I know it's unacceptable but I don't tip at all if the service is poor. I usually tip about a dollor per each ten dollars spent but again only if the service is good. If I have to sit through half my meal without anything to drink you ain't getting nuttin from me.

If you go to any resturaunt where you are served something than you should tip. I leave 15% of the total bill, because usually at those resturaunts the tips are divided between the staff. I also tip if I order a to-go meal at a resturaunt that you would generally be served.

Only if there are waiters/waitresses who come to the tables to refill or bring us things. At buffets we generally do a $1 a person.

Well most of the time someone is coming to offer water and or tea, a beverage. They may pick up plates as you finish them. Give them a dollar or so, unless you have a large group and lotsa plates and glasses. Go higher then.

It would be nice because they make below minimum wage and have to live off their tips, but as far as percentage I don't know.
