I first heard it said by Cards broadcaster Jack Buck in Spring of 2000 or 2001. I soon heard Red Sox Nation, Tiger Nation etc.
Now it usually refers to Boston, but

The term was used by a Boston Globe writer in 1986.

The term was first used in Football, by the Oakland Raiders. With the teams success, came a huge fan base, all over the US and even around the world. The term was coined to recognize that though fans may be different, we all were One "Nation" in support of our team. Being a Raider fan is more than just being a fan. The Oakland Raiders are one of the few teams in professional sports to have club sanctioned Booster Clubs. These Boosters are all over, including Europe and Asia.

I think the Oakland Raiders fans started the "nation" thing with Raider Nation. Pretty big out here in Bay Area, California. Even when the Raiders go 3-13 HaHaHaHaHaHaHaHaHaHaHa...Red Sox Nation is the real deal now....

First time I ever heard the term was associated with the NFL Oakland Raiders around the same time you first heard it used for the Cardinals.
