It means you're playing against real humans. Developers can control the initial content of the game that gets rated for content by companies like the ESRB, but once you add the human element to the mix they lose a great deal of control over what the player experiences (the difficulty, if anyone is being an idiot and team-killing constantly, if people are swearing or doing other offensive things, etc)

Due to the latency of connections, the game is different online. Movement is different, for shooters, the shooting is different. In reality, everyone is playing a different game, usually at least a couple of milliseconds behind each other. If you play Halo on your XBOX with friends not online, it plays different than if you play online in matchmaking.
For single player, the game is usually updated around 30-60 times a second, for online it's usually updated with all the player's data 1-10 times a second. Lag can make the game experience extremely different.

Say you game is rated T for teen. You go online and suddenly all bets are off. Someone could be doing something that is inappropriate for that rating.
