I have been trying for years to get my act, or my sh1t, together, and have had limited success. I would love to hear from anyone ANYONE who has gotten his/her stuff together, even for a little while, and to hear how he/she did it.

Make a list of short term goals. Short term can be daily and/or weekly items.
Next list long term goals: month, 6 months, year, etc. As you accomplish things in your life and look at the list periodically, it will give you a perspective on your life and as you keep adding and crossing off items, it really does help.
I have mine in a small notepad. Being organized is really difficult for me so I write a daily goal to accomplish.

Don't be nervous. This happens to all in the beginning bIut slowly one gets over it.If somebody boasts to you that he was Don Von at the first encounter take it with a lump of salt. And it is as it should be.Your partner may not like rushing through things. Also do not repeat the contacts to study how you are coming up.Other things being OK just jump in without bothering how deep are the waters.

I did it. Got my sh*t together. I had to ACT on things instead of just thinking about them. Instead of thinking about wanting a degree, I signed up for school to get one. I started to just do the things that would result in what I wanted/goals. I made me feel better.
Then I got knocked down again, so I'm working to get it back again. I realized you will always be thrown curve balls. Once you deal w/ issues, you feel better, but new issues can develop at any time to take their place. But that's OK, it all makes you stronger. You just have to do what you want regardless if you think you may fail or not. You have to be honest with yourself and face your fears. Facing your fears is an amazing feeling! Set goals and don't let anyone get in your way to distract from them.
