I always dream at night and feel tired after I wake up. Does not feel refresh at all. I frequently yawn or even fall asleep during the day.

If you were dreaming more it would mean that you were missing out on sleep!

Dreaming occurs during REM sleep or the lighter sleep although, you need to dream some!

"sleeping through the night" for a baby is 5-6 hours. Your baby is only 4 months old I wouldn't worry about it yet. My son didn't sleep through the night until 6 months. And I too had to be to work by 5 it us difficult but there isn't much you can do. The only thing I can suggest is self soothing. Sometimes babies wake up and just don't know how to fall asleep themselves. Try putting her to bed awake and doing the cry it out method. You already have a good bedtime routine so that should be no problem and lay her down drowsy but awake and let her fall asleep alone. And if she wakes up in the middle of the night give her a few minutes to try to fall asleep again on her own.

Talk to a doctor about sleep breathing disorders-also known as sleep apnea. Typically this problem occurs just as you go into REM, the dream stage of sleep. Only a sleep study can tell if this is your trouble, and it has to be ordered by a doctor.
