It really depends upon how tender your stomach is. I've never had a problem with it, but some people experience acid reflux (heart burn) from taking analgesics on an empty stomach.
It shouldn't make you sick however, and taking it on an empty stomach will accelerate the absorption of the drug, making it work that much faster.

I'm not a doctor, but both tylenol and midol have the same active ingredient acetaminophen. That's a painkiller.
The other active ingredients in Midol are diuretics, which increase urine production.
You can take acetaminophen on an empty stomach, it's not like iboprofen or A.S.A.(asprin), where it eats the stomach lining, but you should try to take more fluids when you're taking midol, since it's forcing more fluids to leave your system.

You can use midol or tylenol at any time of the day but for cramps I think Ibuprofen or Naproxen would be more effective. Also if you cramps are that bad you need to see your doc to be sure you don't have something like endometriosis causing this.

Tylenol side effects
Get emergency medical help if you have any signs of an allergic reaction to Tylenol: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
In rare cases, Tylenol may cause a severe skin reaction that can be fatal. This could occur even if you have taken acetaminophen in the past and had no reaction. i would just eat something small it is not good to take any meds without food or at least drink milk with it. But if you really get bad cramps i would just take midol that is what is made for. so that would work much better.

I think Tylenol is your best bet. Midol never worked for me. I hope you feel better. Don't take a shower with hot hot water it will make you cramp more,

advil is the only thing that works on cramps for me. just eat a bite of bread or something if you are that worried about your stomach. even milk will do. if you are only taking it for that then you shouldn't worry. advil only affects your stomach when you take it several times a day every day.

I would recommend talking to your pharmacist about this as well. As for the really bad cramps I would recommend that you see your dr about them to see if they can help you to figure out what could be causing them.
