If the physical is a direct condition of employment, most employers will pay for it. If it is a condition of a license needed for employment, such as a Commercial Drivers License then the employer may or may not pay for it.

If your employer does not want to pay, that is something that you must work out with them. If you feel strongly enough about it, get a job elsewhere.

If the employer requires it as a condition of employment, then yes, the employer should pay for it, or reimburse the employee for it.
