You don't need any special form. Just state that you will pay $x.xx tomorrow, if they will accept that amount as full settlement of your debt. Do not send the payment until you have a written agreement in hand. Do not make the offer unless you are prepared to pay the full settlement immediately.

As you can see my income does not nearly cover my expenses, however I would much rather pay my debts than claim bankruptcy. So I am therefore asking you to reduce your minimum payment to ----------------.
