It's an old saying like "You can't have your cake and eat it to." Um, why have cake if you can't eat it? I know, but that's another question.
Used to be, most covers were the same, brown or black leather with gold - or possible black, red or brown cloth binding. It had only the title and the author. it told you nothing about the contents.
Now, of course - 90% off all books are sold based on cover art. it is the single most important decision that can be made for a books. thee are various kinds of books- mass market - Trade, Turtle back, hard cover with Jacket... and even more I won't bore you with. They frequently have a stunning scene in the book and a wrody description of the contents and on the inside some critical review and a picture of the author with bio!
So, we've kept the saying around because it's kind of catchy and part of our venacular - even though it is now (mostly) incorrect.

You can't tell a book by its cover cuz its covered by a cover.
It could be anything underneath that cover besides a book.
Like a calculator-size-book or a huge chocolate bar.
So yea the cover can be fancied up with decorated letterings and a nice book Title but always remember its just a cover covering the truth. It is only when you lift up the cover you find underneath the true form of whatever the cover was covering.

You can't tell a book by it's cover literally because the cover might look boring but inside it could be interesting.
In reality the person might look different in the outside but in the inside he/she could have a great personality and a very warm heart :)

Because the cover is part of the marketing strategy, and the intention is "to sell", so very often it doesn't represent the content or the quality of the book.
