I actually just went throught this in Florida. You have to go to your local Social Security office and to the DMV to change your name. You can fill out the paper work (which can be downloaded on their websites) which will save some time. Congrats and good luck.

You have to file paperwork. I lived in Florida when I got married 2 years ago and I believe the first thing I had to do was go to a Social Security Office (which sucked because there was a looooong line) and fill out paperwork. Of couse take your marriage certificate and id. Once you receive your new social security card (which didn't take long) you will have to go to or call all the places that have your name on file as your maiden. Most places want to see your new social security card and/or your marriage license. It was quite a pain in the butt and I think I still have one or two things in my maiden name. Oh well, it is the nature of the beast :)
Good luck and congrats!

How to change your name when you get married
As with so many legal matters, the rules change by municipality, so check with your local city hall. But generally, the steps you should take are:
Before the wedding
Tell your job so they can start the process of changing your email address, and your business cards.
Tell your bridesmaids and family that you're changing it, just in case anyone checks with them before getting anything monogrammed.
Make your honeymoon reservations using your MAIDEN name. You won't have a passport or drivers license in your new name yet, so you'll need your tickets to match your documentation.
After the wedding
Get a new social security card
Once you get your marriage license (which usually takes a couple of weeks to arrive in the mail), download a form from the Social Security website.
Congrats!

No, you have to do alot of paperwork, you must go to the Social Security office, with your marriage license to do this. Whatever paperwork you are changing, be sure to take your marriage licenses.

Unfortunetly, there is no easy way out on this one. You will have to file all the necessary paperwork and then go around having it changed with Social Security, Driver's Licence, etc. Have fun running around, it takes forever it seems to get your name change completed totally.

If you are going to take on your husband's name, you'll need to file for a name change with the government. Then, you'll need the legal documents showing your official name change to take to various places such as your bank, the Department of Licensing, for your new Social Security card, other accounts such as phone bills, brokerage accounts and such. It's a hassle, but it's best to get it all out of the way at once so you don't have any lingering accounts that you forgot to change later on that give you trouble...

The only thing you need is your marriage license and birth certificate. You don't go to the court house or anything like that to make it a legal change. The marriage license is your proof of marriage, which allows you to take your husbands name, but it doesn't obligate you to, you can keep your maiden name if you want to.
You need to take the license and birth certificate to the SS administration first to get a new card. Some license bureaus will change it with just your old license, birth certificate, and marriage license, some want the new SS card too, all you can do is check, same as bank accounts. Some make you wait for your new SS card, some will let you use your license, marriage license. But that is it, as far as legally changing your name, is going to the SS administration with proof of your marriage, birth certificate, your old SS card and photo ID.

If you have never switched anything over that means that you are still your given name. You don't have to take his last name. Unless on your marriage license you put that you wanted his last name. Which in Florida you have to put whether you are going to take his name or hyphenate yours with his or just simply keep yours. Best thing is to look at your marriage license. Are you using a lawyer? You could ask them if so. But Marriage license is the first place I would start.
